dentifygroup.com is provided by Dentally Group, LLC (“Dentify” / “we”) and we protect your personal privacy. We have developed this data protection policy which describes how we collect and use your personal data as visitors to our websites. The policy also describes what rights you have under applicable data protection legislation and how you can enforce your rights.

WHAT INFORMATION DO WE COLLECT?

In order to offer safe and secure dental care, Dentally collects information about you that is necessary to be able to meet your dental care needs. When you visit our websites or contact us, you may therefore provide information about you to us. Below is a list of personal data we process, for what purpose and on what legal basis.

Appointment. We process your personal data when you contact us to book an appointment at Dentally’s clinics in order to administer and confirm your booking. The following personal data is processed; name, social security number, e-mail address and telephone number. In addition to the information just mentioned, we will also register the reason why you contact Dentally, for example due to pain, lost filling or toothache. The treatment is based on your consent. Your consent is voluntary and can be revoked at any time by contacting us by e-mail or regular mail, see the section “Contact us” below. Please note that it is necessary that you provide your personal information in order for us to handle your booking. If you choose not to provide your personal information, this means that we will not be able to book an appointment for you at one of Dentally’s clinics via our booking system.

Journal information and documentation for treatment at one of our clinics. We are required by law to keep a patient record for all our patients. Therefore, we have the right to register your personal information in our patient record even if you have not given your consent. A patient record must always contain the following personal data: Information on public health, background to care, diagnoses, reason for more significant measures, information on measures taken and planned.

Inquiry via email or contact form. We process your personal data when you contact us via email or via our contact form in order to handle your request, with the support of a balance of interests. We believe that we have a legitimate interest in processing your personal data in order to handle your request. The following personal data is processed; name, social security number, e-mail address and telephone number.

Newsletter. If you choose to subscribe to our newsletters, we process your personal information in order to provide you with the newsletters, with the support of a balance of interests. We believe that we have a legitimate interest in processing your personal data in order to handle your subscription. The following personal data is processed; name and email address. You can always choose to cancel your subscription to our newsletters by clicking on the link at the bottom of the newsletter.

Use of our websites and contact forms. We process your personal data when you use our websites and contact forms in order to provide the website in a secure manner and to improve our services, based on our legitimate interest in providing the websites in a secure manner and to be informed about how the website is used. The following information will be processed:

  • Information about the use of Dentallygroup.com – such as visitor statistics about response time for pages, download errors, how you reached and left the service, as well as delivery notices when we contact you.
  • Device information – e.g. IP address, language settings, browser settings, time zone, operating system, platform and screen resolution.
  • Geographic information – your geographical location. We use Google Analytics and collect data in accordance with the user settings that you as a user have specified in your settings in your browser. They handle only the necessary information and in accordance with the applicable data protection requirements. RECIPIENTS AND CATEGORIES OF RECIPIENTS

Only those people at Dentally who need to know your personal information in order to perform their duties have access to it.

We may share your personal information with Dentally’s clinics so that they can offer you safe and secure dental care. We may also share your personal information with suppliers who perform services on our behalf, such as an external accounting firm, suppliers who provide information storage services and suppliers of dental materials, clinical equipment. To the extent permitted or required by applicable law, we may share your personal information with authorities, courts and other similar third parties.

Your personal data will always be processed within the US, unless otherwise expressly stated.

STORAGE PERIOD

The contact information that we collect in connection with appointments and contact forms is stored in our patient record system in accordance with the provisions of the Patient Data Act. Regarding newsletters, your information is stored in our database as long as you choose to receive our newsletters.

YOUR RIGHTS

  • Right of access. You have the right to receive confirmation from us as to whether personal data about you is being processed. In such cases, you have the right to access your personal data as well as information about, among other things, the purposes of the processing, the categories of personal data to which the processing applies, and the recipients or categories of recipients to whom your personal data has been or will be disclosed. You have the right to receive a copy of the personal data processed. The copy is free to request. For any additional copies you request, we may charge a reasonable fee based on our administrative costs

  • Right to erasure (“right to be forgotten”). In certain circumstances, you have the right to have your personal data deleted.

  • Right to object. In certain circumstances, for reasons relating to your specific situation, you have the right to object to our processing of your personal data at any time. We may then be obliged to stop the processing of your personal data.
